2023-24 Life Skills screenerThank you for being our valued partners in educating CK Schools students. As you may already be aware, students require specific relational life skills to help them succeed both in school and in life. We want to better understand how to best support those skills.

Between October 10 and October 25, students will have the opportunity to self-assess their life skills in the Life Skills screener. This online screener will take six minutes for grade 5 and nine minutes for grades 6-12. 

Students are not required to take the screener.

Parents/guardians can complete an online form through PowerSchool (see more below) or pick up a paper form from your school’s main office to opt your child out of the Life Skills screener.

To complete the PowerSchool form:


1. Log into the PowerSchool Parent Portal.
2. Select your student's name in the blue bar at the top of the screen.
3. Click “Forms” in the left menu.
4. Click on the “CKSD Student Life Skills screener Information with Opt-Out Form” link.
5. Fill in all required fields.
6. Click the submit button.
7. Repeat steps 3-7 for all students.

We will ask students in grade 5 to reflect on:

  • supportive relationships

You can preview survey content below. The survey takes between 7-15 minutes to complete.

We ask all students to participate in our screener and surveys. Their responses provide invaluable insights into how we can improve and adapt our district to their needs. 

Based on data from last year’s Life Skills survey, CK Schools committed to act on the following themes: 
  • Promote youth voice and engagement: We plan to increase communication on how student feedback is being used. We will develop or expand student voice opportunities and expand student-led advocacy opportunities in schools.
  • Foster supportive school and classroom relationships: We want our schools to have a strong sense of community. Our schools will create structures that foster belonging and emotional safety. We will also establish student discipline that supports skill and relationship development.
  • Seek authentic family partnerships: We plan to increase opportunities for two-way communication and opportunities for families to meaningfully contribute to decision-making processes. We also plan to improve communication to families about school programs and activities.
If a student doesn’t feel like they have enough information to answer a question, they can skip it. Responses to these surveys will be completely confidential. We partner with a third-party vendor to support us in administering these surveys.
