  • Vaccination card or photo of card

  • Documentation from health care provider

  •  Washington State Immunization Information System (IIS) record

To verify your vaccination status:

  • Email proof of vaccination to Include your legal first and last names and your work location. You will receive an email reply once verified.

  • Show proof of vaccination in person at Human Resources (located at the Jenne-Wright Administration Center) between 7:30 a.m. and 4:30 p.m. You can stop by one of the outside HR windows in the back parking lot (clearly labeled), or you can enter the building and visit the office.
