Online Form Instructions
  1. Go to

  2. Sign in or create a new account

  3. Click “School Choice Application 2019-2020”

  4. Start a form by clicking the name of a student OR If you don’t see any student names listed, enter the information for your student and click “Add Student”

  5. You may be asked to import data from a past form. Choose whether you want to “Import” (recommended) or “Start from Scratch” and click “Continue.”

  6. Read the form instructions on the “Introduction” page and click “Next.”

  7. Enter your student’s information into the application form and click “Next.”

  8. If you are missing required information, you’ll see a red “required” warning in the “Review” section. Click “Edit” to update the form.

  9. Once you have reviewed your information and it is correct, click “Submit.”

  10. If you live out of district, make sure to fill out a Choice Transfer Request form for your home district as well.

  11. Don’t forget! Submit a form for each family member applying for a transfer.

  12. Save and Sign Out by clicking the circle in the top right of the page.

  13. You’re done!
